What is a periodontal pocket?

A periodontal pocket is the space between the gum and the tooth. Everyone has one around every tooth; in health it is a shallow groove called the sulcus, 1–3mm deep, with the gum sealed firmly to the tooth at the bottom. When plaque and calculus collect in that groove and the gum becomes inflamed, the attachment at the bottom breaks down, the bone underneath starts to resorb and the groove deepens into a pocket of 4mm, 5mm, 6mm or more.

A deep pocket is a problem because it is self-perpetuating. Once it is deeper than about 3mm, a toothbrush and floss cannot reach the bottom, so plaque and calculus keep accumulating there, the inflammation continues, and the pocket keeps deepening while more bone is lost. Why is a periodontal pocket sometimes called a “periodontal bag”?

How is a periodontal pocket measured?

Pocket depth is measured with a periodontal probe, a slim instrument with millimetre markings that is slid gently between the gum and the tooth until it meets resistance. Does the pocket close after cleaning?

A pocket becomes shallower after cleaning, but it does not return to what it was before the disease. What does not change is the bone. Bone destroyed by periodontitis does not grow back after pocket cleaning; the treatment stops the loss where it is.
