Tooth extraction is our last resort.

When tooth removal is genuinely needed — including wisdom teeth — we make it as gentle as it can be. But first, we'll do everything we can to save your natural tooth.

Save the tooth first. Pull it last.

Some clinics treat extraction as the easy option. We don't. A natural tooth — even a damaged one — is almost always better than any replacement. Before we ever recommend pulling a tooth, we look at every alternative:

  • Can a filling repair the decay?
  • Would a root canal save the tooth?
  • Could a crown protect what's left?
  • Is there gum disease we can treat?

Only when the tooth genuinely can't be saved — or it's putting other teeth at risk — do we recommend tooth removal. And we'll explain exactly why.

Extraction should be the last option, not the first one offered.

Tooth removal types

Two kinds of extraction.

Most extractions fall into one of two categories. Dr Belal handles both in our Berala clinic.

Simple extraction

For a tooth that's fully visible above the gum line. The tooth is loosened with an elevator and removed with forceps under local anaesthetic. Most cases take 20 to 40 minutes.

Surgical extraction

For teeth that aren't easily accessible. A small gum incision allows access; the tooth may be removed in sections. Includes most wisdom tooth extractions and broken-off teeth.

Dr Belal examines the tooth and takes an X-ray to see the roots and surrounding bone. We discuss whether the tooth can be saved or whether extraction is the best option.

Local anaesthetic

A small numbing injection so you don't feel pain. We wait until you're fully numb. Happy gas (nitrous oxide) is available if you're anxious — it takes the edge off without putting you to sleep.

For a simple extraction we loosen the tooth with an elevator, then lift it out with forceps. Surgical cases involve a small gum incision and removal in sections. You'll feel pressure, not pain.

😌 For nervous patients

Happy gas available.

If the thought of having a tooth pulled makes you anxious, you're not alone. We offer nitrous oxide (happy gas) alongside local anaesthetic for nervous patients. You'll be awake and aware, but relaxed and comfortable. The effects wear off within minutes after we stop the gas — you can drive yourself home.

Dr Belal has 20+ years of experience working with anxious patients. We never rush, and we explain every step before it happens.

⚠️ Avoid dry socket

Dry socket: what it is & how to avoid it.

Dry socket happens when the protective blood clot in the extraction site dislodges or fails to form, leaving the bone exposed. It's painful and slows healing. It affects only 2-5% of routine extractions, but up to 10% of lower wisdom teeth removals.

⚠ For 72 hours after, AVOID:

  • Smoking and vaping (suction dislodges the clot)
  • Drinking through straws (any suction is bad)
  • Vigorous rinsing or spitting in the first 24 hours
  • Hot drinks, alcohol, and spicy foods
  • Strenuous exercise that raises your heart rate
  • Poking the area with your tongue or finger

✓ DO:

  • Bite firmly on gauze for 30 minutes after extraction
  • Keep your head elevated when resting
  • Use a cold pack on the cheek (15 on, 15 off) for the first 6 hours
  • From day 2: gentle warm salt-water rinses (let liquid fall out, don't spit)
  • Stick to soft, cool foods — yoghurt, mashed potato, scrambled eggs
  • Take pain relief on schedule, not when pain peaks

If you experience throbbing pain that worsens 2-4 days after extraction, an unpleasant taste, or pain that radiates to your ear — call us straight away. Dry socket is treatable, but the sooner the better.

Recovery timeline

Healing, day by day.

Most patients heal smoothly. Here's what's normal at each stage.

First 24h Blood clot forms. Light oozing is normal. Don't rinse, spit, smoke, or use straws. Bite on gauze for 30 minutes.
Days 2-3 Swelling peaks. Soreness and tightness are normal. Cold packs help. Begin gentle salt-water rinses.
Days 4-7 Pain easing. Healing tissue starts filling the socket. Most people return to desk work after 2-3 days.
Week 2 Surface healing. Eating and talking feel easier. Stitches (if used) dissolve or are removed.
Weeks 3-6+ Bone healing. The socket fills with new bone underneath. The "hole" looks smaller and finally closes.

If you have private health insurance with general dental cover, your fund will usually contribute toward the cost of a tooth extraction or wisdom tooth removal. We can run a quote with your fund before your appointment so you know your out-of-pocket cost upfront.

FAQ

Common questions.

No — the procedure itself is performed under local anaesthetic, so you only feel pressure, not pain. For nervous patients we also offer happy gas (nitrous oxide) to help you stay calm and comfortable. Some soreness and swelling for 2 to 4 days afterwards is normal and easily managed with regular pain relief.
A simple extraction usually takes 20 to 40 minutes including the time for anaesthetic to take effect. Surgical extractions and impacted wisdom teeth take longer — typically 45 to 90 minutes. Multiple teeth in the same area can often be done in one visit.
Often, yes. Even badly damaged teeth can sometimes be saved with root canal treatment, fillings, or crowns. We exhaust every option before recommending extraction — including discussing root canal therapy where possible. Dr Belal will only recommend extraction if your tooth genuinely cannot be saved or is putting other teeth at risk.
Yes. Dr Belal removes most wisdom teeth in our Berala clinic, including impacted ones. For very complex impactions — particularly lower wisdom teeth lying close to the inferior alveolar nerve — he may refer you to an oral surgeon for safety. He'll explain clearly why if a referral is the right call for your case.

Dry socket affects only 2 to 5 percent of routine extractions but up to 10 percent of lower wisdom teeth extractions.
Most patients feel back to normal within 3 to 7 days for a simple extraction, and 7 to 14 days for surgical extractions or wisdom teeth. The gum surface usually closes within 10 days; deeper bone healing continues for several weeks underneath. We provide written aftercare instructions and check in with you to make sure healing is on track.
Every extraction is different. Simple extractions are quicker and less expensive than surgical or impacted wisdom teeth removal. The cost depends on the tooth, complexity, and whether sedation is needed. We provide a clear written quote at your assessment before any treatment. Most extras health funds with general dental cover contribute toward extractions.
Stick to cool, soft foods for the first few days — yoghurt, mashed potato, scrambled eggs, smoothies (eaten with a spoon, no straws), well-cooked pasta, and soups that aren't too hot. Avoid crunchy foods, nuts, chips, popcorn, seeds, and spicy foods until your dentist says the site is stable. No straws and no smoking for at least 72 hours.

Wisdom teeth — do yours need to come out?

Keep them if…

  • They've come through fully and straight
  • You can clean them properly
  • No pain, swelling, or recurrent infection
  • They're not pushing other teeth

Remove them if…

  • Impacted (stuck, growing sideways)
  • Repeated infections or swelling
  • Causing cavities in the tooth in front
  • Damaging neighbouring roots (visible on x-ray)
  • Orthodontist recommends removal

We'll take a panoramic x-ray to see all four and give you a straight answer. If it's complex, we refer to a trusted oral surgeon.

What to expect after an extraction.

First 24h

Bite on gauze 30–60 min. Soft foods. No rinsing, no straws, no smoking — you'll protect the clot. Mild bleeding/ooze is normal.

Days 2–3

Peak swelling. Ice 20 min on/off. Warm salt water rinses after meals (gentle). Painkillers as directed.

Day 7+

Back to normal. Full gum healing takes 4–6 weeks. Bone fills in over months. Call us if pain gets worse after day 3 (possible dry socket).

Common questions.

Will it hurt?
During the procedure — no. You'll feel pressure and pulling, not pain, once properly numb. Afterwards, expect moderate ache for 2–3 days (worse for surgical/wisdom), well controlled with paracetamol + ibuprofen taken together (yes, you can — safely, ask us for the combo). If pain gets worse after day 3 rather than better, call us — could be dry socket, which is very treatable.
How long until I can eat normally?
Soft foods for the first 3–5 days — yogurt, scrambled eggs, mashed potato, pasta, soup (not hot). Avoid crunchy, crumbly, or sticky foods that can get lodged in the socket. After a week you can chew on the other side normally. Full return to chewing on the extraction side: 2–3 weeks for simple, up to 4 weeks for surgical wisdom.
Do I need time off work?
For a simple extraction, most people go back the same day or the next. For a surgical extraction or wisdom teeth, take 1–2 days off if you can — especially if your job is physical or you'll be around others. Desk work is usually fine after 24 hours. We'll write you a medical certificate if needed.
Can I drive home after?
After local anaesthetic only: yes, you're fine to drive. After happy gas (nitrous): give it 15–20 minutes and you're clear to drive. After IV sedation (specialist referral only): no — you'll need someone to drive you home and stay with you for 24 hours. We'll confirm this before you book.
What if I'm terrified?
Tell us when you book. Seriously. Many of our patients started out terrified — from childhood experiences, previous bad extractions, or just general anxiety. We plan around it: longer appointment, nothing rushed, happy gas available, breaks whenever you want. For severe anxiety we can refer you for IV sedation with a specialist. There's no "toughing it out" required — let us handle the comfort so you can handle the appointment.
Should I replace the tooth after?
For wisdom teeth: no, they don't need replacing — they're not functional teeth. For other teeth: usually yes, especially if visible or used for chewing. Missing teeth cause the neighbours to drift and the opposing tooth to over-erupt, which creates problems down the track. Options are implant, bridge, or partial denture — we'll walk you through the trade-offs honestly when you're ready. No rush: most replacements can be planned 2–3 months after the extraction has fully healed.
