Sore gums around one tooth: what it means

General Dentistry

Sore or irritated gum around one tooth: food impaction, a rough filling, a pocket, a wisdom tooth or an abscess. What settles in 48 hours, what does not.

Sore gums around one tooth almost always have a local cause: food packed between the teeth, a periodontal pocket, a rough filling margin, an erupting wisdom tooth, or an abscess. Gum irritation across many teeth is usually gingivitis or gum disease. Soreness that settles within 48 hours of careful cleaning was probably trapped food; soreness that does not needs assessing.

Quick facts: Berala Dental is a family dental practice at 184 Woodburn Road, Berala NSW 2141, operating since 2002. Sore or irritated gums around a single tooth are assessed at a focused consultation for one specific problem for $80 with an oral health therapist or Dr Mahnaz Khan, or at a check-up, X-rays and clean for $250 with X-rays, photos, a periodontal screening and fluoride treatment included. Where a periodontal pocket or gum disease is the cause, a deep clean is $400 per quadrant or $1,600 for the full mouth, with maintenance appointments at $400 each afterwards that are not included in the $1,600. Every fee assumes the health fund contributes nothing, and a written quote is given before treatment. Berala Dental is a Bupa Members First Platinum and nib First Choice provider, accepts all Australian health funds and processes HICAPS on the spot. It is a general dental practice with no specialist periodontist on site, and advanced cases are referred.

Why would the gum around just one tooth be sore?

Because something is happening at that one spot that is not happening anywhere else. Gum tissue responds to plaque the same way all around the mouth, so when only one patch is sore, red or puffy, the explanation is nearly always a physical irritant at that site: something wedged, something rough, something infected or something pushing through. Whole-mouth causes such as gingivitis, pregnancy or a medication do not pick a single tooth.

A single-site problem is investigated at Berala Dental at a focused consultation for $80 with an oral health therapist or Dr Mahnaz Khan; gum irritation across many teeth is better assessed with a full check-up, X-rays and clean for $250.

What are the common causes of sore gums around one tooth?

Five causes cover almost every case of localised gum irritation seen at Berala Dental, and each leaves slightly different clues.

How is sore gum around one tooth different from sore gums everywhere?

Generalised gum irritation — tenderness, redness or puffiness along most of the gumline — is a whole-mouth response to plaque. In most people that is gingivitis, fully reversible with a professional clean and consistent daily cleaning. In some it is periodontitis, where inflammation has reached bone and pockets deeper than 3mm surround many teeth. Pregnancy, some medications, dry mouth and poorly controlled diabetes produce the same generalised picture.

A single sore site is examined, X-rayed and treated at that site. Generalised soreness is measured with a periodontal screening across every tooth, included in Berala Dental’s $250 check-up, X-rays and clean, to separate the reversible gum-only stage from the stage that has reached bone. The two stages need different treatment, set out in what each stage of gum disease actually needs.

What settles in 48 hours, and what does not?

Two days is the dividing line Berala Dental uses when someone phones to ask whether they need to come in.

Likely to settle within 48 hours of careful cleaning: soreness from a food fragment that has now been flossed out, mild irritation from a hard toothbrush, a small scratch from a crust, and the first few days after starting to floss again.

Will not settle on its own: soreness beside a broken filling margin, a periodontal pocket, a wisdom-tooth flap, or any abscess. These calm down for a week or two and flare again in the same spot. Recurring soreness at the same tooth is the pattern to take seriously, because bacteria have been sitting there the whole time and bone lost during that period does not grow back; treatment stops the loss where it is.

Sore in the same spot for the second or third time? That is the point to have it measured.

When is a sore gum around one tooth urgent?

Most localised gum soreness is a same-day dental problem, and Berala Dental keeps same-day emergency appointments for it. A small number of cases are infection spreading beyond the mouth, and those go straight to a hospital emergency department.

A bump that drains, a tooth that throbs, or a gum that bleeds and aches is uncomfortable but not usually dangerous. Those are treated at Berala Dental, 184 Woodburn Road, Berala, and every call from someone in pain is treated as urgent.

What does a dentist do for sore gums around one tooth?

The appointment has a fixed order: find the cause, relieve it, then stop it coming back. A dentist or oral health therapist at Berala Dental looks closely at the gum and the tooth beside it, runs a periodontal probe around the tooth to check whether the pocket is deeper than 3mm, checks the filling or crown margin with a fine instrument and floss, and takes an X-ray to see bone level and whether the tooth itself is the source.

Treatment matches the finding. Trapped food is removed and the contact point restored so it stops happening. A rough filling margin is smoothed or the filling replaced. A periodontal pocket is cleaned below the gumline under local anaesthetic. A wisdom-tooth flap is cleaned beneath and the tooth assessed for removal. An abscess is drained and the tooth treated with root canal treatment or an extraction. Antibiotics are sometimes prescribed alongside one of these steps when infection is spreading, but never instead of them, and only the treating dentist or doctor decides.

What does it cost to have sore gums around one tooth assessed at Berala Dental?

All figures assume the health fund contributes nothing, and a written quote with item numbers is provided before any treatment beyond the assessment.

What a health fund contributes is set by the fund and the policy, so the fund is the right place to ask before the visit.

What can you do at home for a sore gum around one tooth?

Clean the sore site rather than avoiding it. Floss gently between the sore tooth and its neighbours to dislodge anything wedged there, brush the gumline with a soft brush, and rinse with warm salt water after meals. Over-the-counter pain relief can be taken as directed on the packet. Do not pick at the gum with a toothpick or pin, and do not squeeze a bump. If the soreness has gone within 48 hours, it was almost certainly trapped food. If it has not, or keeps coming back at the same tooth, book an assessment.

Quick answers

Why is the gum around one tooth sore but not the others?

A single sore site nearly always has a local physical cause: food wedged between the teeth, a rough filling margin, a periodontal pocket, a partly erupted wisdom tooth or an abscess. Whole-mouth causes such as gingivitis do not single out one tooth. Berala Dental assesses one sore site at a focused consultation for $80 with an oral health therapist or Dr Mahnaz Khan.

How long should a sore gum around a tooth last?

Soreness from trapped food or a minor scratch settles within about 48 hours of careful cleaning. Soreness lasting longer than two days, or returning to the same tooth, points to a pocket, a broken margin, a wisdom-tooth flap or an abscess, none of which resolve on their own.

Can a filling cause gum irritation around a tooth?

Yes. A filling or crown with a rough, cracked or overhanging margin traps plaque against the gum where a brush cannot reach, and the gum beside it stays sore and bleeds. Floss that shreds or catches at one tooth is the usual sign. Smoothing or replacing the filling usually resolves it.

Is gum irritation around a tooth a sign of gum disease?

It can be. A periodontal pocket deeper than the healthy 1–3mm at a single tooth is localised gum disease; the soreness flares and settles in cycles while bone is lost silently underneath. A periodontal probe and an X-ray at Berala Dental confirm it in one visit, and a deep clean of that area is $400 per quadrant.

What should you do next about a sore gum around one tooth?

Floss the site, give it 48 hours, and check the four hospital signs. If it is still sore after two days or has come back at the same tooth, have it examined. Berala Dental has assessed sore gums at 184 Woodburn Road, Berala, since 2002, and no question is a silly one.

Berala Dental has looked after Berala families since 2002, six days a week including Saturday.
